A wide reaching service that provides ongoing guidance and support to individuals returning from a stay in a psychiatric hospital to help maintain their safety and transition back into everyday life, provide additional care to those who are working with a community team but who have more specific and detailed requirements, and to those who are currently unsupported because they are awaiting a diagnosis or are unable to leave their homes.

This support might include safety and risk assessments, psychoeducation for friends and family, advice on transitioning between hospitals, institutions and everyday life with mental health issues, managing finances, finding and keeping jobs, transport, activities and hobbies and much more.

Programs designed specifically around you and your requirements to ensure you get the most from life and are not held back by illness or disability.
